چکیده

A case is reported of jaundice developing in a patient with obstruction of the left hepatic duct and partial obstruction of the right hepatic duct by tumour. Adequate drainage of bile from the right hepatic duct through a T-tube was relatively ineffective in relieving the jaundice. On the other hand, resection of the obstructed left lobe resulted in the disappearance of icterus. The basic mechanism involved in the production of jaundice under these circumstances is incompletely understood.

We report a case requiring biliary reconstruction with right hepatic lobectomy due to biliary strictures caused by continuous cholangitis after laparoscopic bile duct injury. The patient, a 55-year-old woman, underwent laparoscopic cholecystectomy for cholelithiasis at another hospital. Although a bile leakage from the intraabdominal drain was observed several days after the operation, the pati...

Successful Surgical Treatment of Hepatocellular Carcinoma Invading Into Biliary Tree

A 41-year-old woman was admitted to hospital with obstructive jaundice. Computed tomography showed a large mass in the right hepatic lobe and marked dilatation of the biliary tree in the left lateral segment of the liver. Angiography showed evidence of neovascularity. Percutaneous transhepatic cholangiography revealed complete obstruction of the common bile duct just below the bifurcation. The ...

Double-guidewire technique using a novel ERCP cannula for hepatic hilar obstruction.

Endoscopic biliary drainage (EBD) for hepatic hilar obstruction is sometimes challenging. Moreover, when unilateral biliary drainage has already been performed, if another intrahepatic bile duct also needs to be drained, EBDmay be mademore difficult because of tumor ingrowth and the narrowed angle from which the bile duct must be accessed. In this case, percutaneous biliary drainage (PTBD) or e...

A case of recurrent cholangitis after bile duct injury during laparoscopic cholecystectomy: value of scintigraphy with Tc-99m GSA and hepatobiliary scintigraphy for indication of lobectomy.

A 39-year-old woman with acute cholecystitis and gallstones underwent laparoscopic cholecystectomy. She suffered from recurrent episodes of cholangitis due to injury of the major bile ducts during laparoscopic cholecystectomy. Hepatobiliary scintigraphy with Tc-99m Sn-N-pyridoxyl-5-methyltryptophan was performed. Although normal bile excretion was found from the left hepatic duct to the percuta...
